When designing a footer and a header for your page, you need to define styles for them. Use the STYLE definitions to specify their positions on the page as well as their widths. Their height will be automatically computed. The following headerstyle and footerstyle classes are examples for header and footer style definitions:

.headerstyle
{
  position:absolute;
  top:.25in;
  width:5.5in;
  left:1in;
}

.footerstyle
{
  position:absolute;
  top:10.5in;
  width:5.5in;
  left:1in;
}
To position a header or a footer in a DIV element, for example, you would go:

<DIV CLASS="headerstyle">Header's HTML goes here</DIV>
<DIV CLASS="footerstyle">Footer's HTML goes here</DIV>
